This item drops from the last boss of volume 2, Book 6 chapter 8. The boss always drops either the signed cloak or the cloak of shadow and flame, he mostly drops the signed cloak. Some people claim that the Cloak of Shadow and flame drops only when the boss dies while he is switching from shadow form to flame form. This has never been confirmed or denied by any dev although there are some hints in the item the description that may point out that this is true.
